Truescore
The instrument behind both practices. Four dimensions, an evidence standard every finding has to clear, and a cycle that restates the position rather than reprinting it.
Three rules, applied without exception.

Traceable
Every finding resolves to a source record. The claim, the encounter, the code, the date. A number that cannot be walked back to the record that produced it is not reported, however useful it would be.

Restated
Positions are re-established on a fixed cycle against the same method, so what you manage is movement rather than a sequence of unrelated snapshots. When the method itself changes, we restate the prior period too and show both.

Independently closed
Closure is confirmed in the data, not asserted by the person accountable for it. The distance between reported closure and evidenced closure is reported as its own finding.

Establish
Baseline
We take one period of data from the contracts you hold and establish your position across all four dimensions. This is the baseline, and it is the deliverable you see before you commit to anything further.

Evidence
Working papers
Every finding is attached to its source record and to what was done for that member in the prior period. This is the part that makes a finding arguable, and being able to argue with it is the point.

Work
Your team
Findings become the prepared page a provider carries and the daily list your staff works. Nothing here requires a clinician to enter a second system.

Restate
Next cycle
The position is re-established against new data. What closed, what did not, what moved, and what the reported closure rate was against the evidenced one.
What Truescore will not assert.
An assurance method is defined as much by what it declines to say as by what it reports.
A diagnosis
We report what the record supports and what it does not. The diagnosis belongs to the clinician, and no output of ours should be read as making one.
A closure we cannot see
If the encounter, code or claim does not carry it, we do not count it, regardless of what a status field says.
A forecast
Predictive and rising-risk modeling, suspecting logic and assisted documentation are on our roadmap. They are not part of Truescore today and nothing on this site should be read as offering them.
A number without provenance
Every figure we publish, here or in a deliverable, carries the source that produced it. Where a figure is modeled rather than measured, it is labeled as modeled every time it appears.

Publishing the method in full is a deliberate trade. It means a competent team could read this page and attempt it themselves, and some should. What it buys is that you do not have to take a single claim on this site on faith before you have seen it run on your own data.
